The Certified Nursing Assistant (CNA) provides personal care services for the patient. The CNA works under the clinical direction of the RN Case Manager for each patient/family.

Primary responsibilities include:

Assisting patients with a variety of personal care tasks
Taking vital signs, changing dressings and helping patients take their medication
Assisting patients/families with light housekeeping duties such as preparation of meals or changing bed linens
Reporting any changes in a patient’s condition to the RN Case Manager
Submitting CNA visit records in a timely manner
Attending and participating in regular meetings

This role requires a graduate from an accredited Certified Nursing Assistant program. Previous experience as a CNA is required. Experience in hospice, home health, and long-term care is preferred. A valid driver’s license and current automobile insurance are required.
